Which pathway is right for you?

All appointments begin with an initial evaluation — €350 for all age groups. Select the pathway that matches the age of the person being seen. This will tell you exactly what to expect, what the full pathway involves, and what the total fees are before you commit.

All initial evaluation appointments are €350 regardless of age group. Full pathway fees vary and are itemised on each age-group page. See also the full fees page.

What to expect

Your initial evaluation — step by step

All NTTC pathways begin with an initial evaluation appointment. This is a comprehensive assessment — not a brief screen. Allow approximately 1.5 to 2 hours. Here is what happens.

01 Physiotherapy assessment

A Chartered Physiotherapist assesses oral function, tongue mobility, jaw movement, cranial and fascial tension, and whole-body posture. For infants, feeding mechanics and latch are observed.

02 Feeding assessment

For infants and young children, an IBCLC assesses feeding at the breast or bottle — latch quality, transfer, maternal nipple symptoms, infant weight gain, and feeding behaviour.

03 Medical consultation

Dr. Justin Roche reviews the full clinical picture — history, examination findings from both previous assessments, and his own examination. Diagnosis and a treatment plan are discussed with you at this stage.

04 Plan and next steps

You leave with a clear diagnosis, a recommended treatment plan, an outline of the pathway ahead, expected timeline, and full fees — before committing to anything further.

Preparing for your appointment

What to bring and what to know

A little preparation makes the appointment run more smoothly. Below is a checklist by patient type.

👶 Infants — breastfeeding
Plan to feed during the appointment if possible — we want to observe feeding directly
GP or public health nurse referral letter if you have one (not required)
Baby's red book / growth chart
Any previous tongue tie assessments or reports
Arrive without having just fed — a slightly hungry baby feeds more actively
🍼 Infants — bottle feeding
Bring your usual bottle and teat so we can observe your normal feed

Note current formula / volume / frequency
Any reflux or medication history
Arrive slightly hungry if possible
🧒 Children
GP, SLT, orthodontist, or dentist referral letter if available
Previous speech and language therapy reports
Any orthodontic or dental records
A snack that your child typically struggles with is helpful for observing chewing and swallowing
Prepare your child in advance — a brief, calm explanation helps
🧑 Adults
GP referral letter if available (not required)
List of current symptoms in your own words — duration, severity, what makes them worse
Any previous orthodontic, ENT, TMJ, or sleep study reports
Medication list
Previous tongue tie release or failed release details if relevant

Evaluation appointments are available at both locations. All treatment and surgical appointments are at Clonmel. The Naas clinic operates on Saturdays only.

Common questions

Before you book

Do I need a GP referral to book? +

No. Self-referral is accepted for all age groups. A GP or other professional referral letter is helpful but not required. If you have relevant reports — from an SLT, IBCLC, orthodontist, or lactation consultant — bring them along to the evaluation, but their absence will not delay your appointment.

What does the initial evaluation cost? +

The initial evaluation is €350 for all age groups. This covers the physiotherapy assessment, feeding assessment where relevant, and the medical consultation with Dr. Roche. If treatment is recommended, the pathway fees are outlined in full at the end of your evaluation appointment before you commit to proceeding. Full fee information is available on our fees page.

How long will the appointment take? +

Allow 1.5 to 2 hours for the initial evaluation. The appointment involves three sequential components — physiotherapy assessment, feeding assessment where relevant, and medical consultation — so it is comprehensive. Please do not schedule anything immediately after it.

Can I book at Naas if I want treatment? +

Yes. The Naas clinic is available for initial evaluation and assessment appointments on Saturdays. Where treatment is recommended following your Naas evaluation, it is performed at the main NTTC clinic in Clonmel. The same clinical team — including Dr. Roche — sees you at both locations.

What if I had a tongue tie release elsewhere that didn't work? +

We see a significant number of patients who have had a previous release that did not resolve their symptoms, or where reattachment has occurred. Please bring any previous assessment or surgical reports. A full re-evaluation is required before any revision procedure can be considered. See our failed release and revision page for more detail.

Is there insurance cover for tongue tie treatment? +

Some procedures may be partially covered by health insurance depending on your provider and policy. We recommend contacting your insurer directly before your appointment to check your entitlements. We provide itemised receipts for all appointments. Full details are on our fees page.

Can I speak to someone before booking? +

Yes — our free Concerns Call is designed exactly for this. A member of the clinical team will speak with you for approximately 15 minutes, listen to your concerns, and give you honest guidance on whether an evaluation is likely to help.
